All of my patients should familiarize themselves with my practice policies. If you do so, you will experience less frustration, and know what to expect when you need something done.

Please note that these are my personal practice policies. They are not necessarily the policies of the other physicians in my practice.

No Shows When a patient shows up late to an appointment, or does not show up to an appointment.
Controlled Substances A controlled substance is a medication which is potentially addicting.
Forms If you have a form which needs to be filled out by the physician.
Prior Authorization Some medications require prior authorization.
Refills When your medication needs to be refilled.
Telephone Medicine When you request treatment over the telephone.
Referrals When you need a referral to a specialist or for a test.
Appointment Length The 15-minute Appointment Slot
Orders Patients who live in group homes or at assisted living facilities often need orders signed.
